Instructions:

(1) Complete this page of the L.I.F.E Sports Academy Scholarship application

(2) Attach the first 2 pages of your 2018: 1040, 1040a, or 1040EZ tax return (omit all schedules and forms)

(3) All applicants must complete the Registration Paperwork before final decision can be made on amount of scholarship provided.

A representative from our staff will be in contact with you within 48hrs of submission.
